Case Manager, Geriatrics Clinic
Job Description
Under the supervision of the Clinics Administrator, the Case Manager coordinates the care of a patient with insurance, provider, state agencies, home health, other facilities, etc. The Case Manager maintains compliance with State professional agencies. The Joint Commission, insurance regulations and all Federal and State standards of care. Case Management is a collaborative process which assesses, plans, implements, coordinates, monitors and evaluates options and services to meet the patient's needs through communication and available resource to promote quality cost-effective outcomes. The Case Manager will perform a wide variety of high-level patient care activities to include Case Management assessment, screening for appropriateness of admission, advising patients ' families on POA or guardianship, development of care plans and patient and family teaching for selected patients. Much of the Case Manager's focus will be on crisis intervention, counseling, education, advocacy, and support of both the patient and patient's family or care givers. This may include assistance and working as a liaison for the patient and their family regarding housing or relocation or housing, money management, arraignments of in home services or care placements.

Qualifications
Master's degree in Social Work with current Iowa license or Master's Degree in Human Services. Minimum of two (2)-plus years of experience. Experience with care planning. Strong professional level knowledge of comprehensive clinical assessment skills in the adult population and experience with chronic disease management preferred. Experience offering crisis intervention, counseling, and support of the geriatric population.
